This form is for any participant who wishes to report an INTERPERSONAL CONSENT CONCERN or INCIDENT before or after London Decompression 2023, including reports about anyone who is due to attend.

You can also email the Consent Leads via consent@londondecom.org - but this online form will guide you through the required responses to make a clearer report.

If your report is directly about one of the Consent Leads, do NOT use this form. Make your report to the Site Lead (who can be contacted during Decom via Radio which are carried by Rangers and Welfare) or email siteleads@londondecom.org.

Neither this form nor consent@londondecom.org will be checked during the Decom event, if immediate action is required, especially if there is a risk to life or limb YOU MUST FIRST REPORT THIS TO A RANGER OR THE SITE LEAD. They can be contacted via radios which are located at Welfare and Gate (when open).

All reports will be reviewed by a Consent Lead and a decision made if further action and/or escalation is required.

Depending upon the length of your report, this form should take approximately 15 minutes to complete.

If your report is about one of the Consent Leads (CK, Gareth and Tina) DO NOT use the Consent email or the online form. Instead you should contact Event Co-Lead Rosa rosa@londondecom.org.

Subject - the person of concern or the culprit of the incident.

Aggrieved - the victim/survivor of the incident.

Reporting Party - the person making the report.
