TO ENSURE A SPACE:
Complete online registration as soon as possible (payment may occur later).

  • Registration / Payment: IAC / Exhibitor registration forms must be completed ONLINE at: https://tinyurl.com/KYDiabetes17Exhibitor. Space is limited and will be allocated based upon completion of the online form on a first-come, first-serve basis. Payment – IAC / Exhibitors may choose to pay online by credit card or by check. If payment must be by check: Make Check Payable To -- Kentucky Diabetes Network (KDN) 2017 Symposium and mail with "Online Registration Form" to: Kentucky Diabetes Network (KDN) Symposium 2017, C/O Green River District Health Department, 1501 Breckenridge Street, Owensboro, KY 42303. KDN is a 501 (C) 3 organization.
  • NOTICE: If CE credit is desired, a separate PARTICIPANT REGISTRATION FORM must also be completed at https://tinyurl.com/KYDiabetes17. Obtaining Continuing Education (CE) Credit – IAC / Exhibitors who are a nurse, dietitian, or pharmacist may obtain CE credit if the representative follows all CE attendance requirements (register online, attend the entire program, complete the evaluation form, etc.). CE space will NOT be reserved for IAC / Exhibitors, however, IAC / Exhibitors may register for CE credit on a first-come, first-serve basis through the “Online Participant Registration Site” at https://tinyurl.com/KYDiabetes17

    IAC may receive continuing education credit for up to 3 company representatives.
    Exhibitors may receive continuing education credit for up to 2 company representatives.

Note:

  • Exhibits will be placed outside the conference room where the program is taking place.
  • The opportunity to be part of  an Industry Allies Council (IAC) or Exhibitor is open to numerous companies that support diabetes care and education.
  • This payment includes exhibit opportunity.
  • Donation of a door prize would be appreciated (may include a company logo or relate to diabetes).

Industry Allies Council Benefits:

  • Pay one fee which includes exhibit fee, meal card, and continuing education credit for up to three (3) company representatives who are either a nurse, dietitian, or pharmacist.   
  • Recognition of level of support announced during the symposium.    
  • Recognition of level of support included in the participant packets (if received by 10-6-17).
  • Recognition of level of support noted on signage displayed prominently at the symposium.
  • Recognition of level of support through table displays.
  • Option to provide a meal or break (may not be the sole provider).

 Exhibitor Benefits:

  • Pay one fee which includes exhibit fee, meal card, and continuing education credit for up to two (2) company representatives who are either a nurse, dietitian, or pharmacist.  
  • Recognition as an Exhibitor included in attendee packet (if received by 10-6-17).
