Extracurricular Activities: Please list the principal extracurricular, sports, community, volunteer, and religious activities that you participated in during the last four years.  Please list them in order of their importance to you.  Include the names of the activity, number of years, and positions held/honors/special recognition.  Maximum of 14 activities. *
Work Experience: List your work experience or paid positions you have held during the past three years beginning with your most recent position.  Include -- Position held; Type of work or duties; Dates (from-to); and Hours per week *
Essay Question: Use some of your own volunteer efforts to explain why community service is important for high school students. How did those experiences benefit the community? How did they benefit you? (500 words maximum) *
