EMS Volleyball Tryout Info Form
Please complete this form for students wishing to tryout for the 2022 Ebenezer Volleyball team. ALL Dragonfly sections MUST be completed before tryouts. Tryouts will take place in the gym May 17th for sixth-grade students only and May 18th and 19th for seventh and eighth-grade students from 3:45-6 pm. 6th graders may be asked to return. This is a closed practice for coaches and players only. Players will need sneakers (volleyball shoes if you have them), knee pads, and water bottles.

(Parents): I understand by the very nature of the sport, volleyball can carry a risk of physical injury. No matter how careful the participant and coach are, the risk cannot be eliminated. The risk of injury includes minor injuries such as muscle pulls, dislocation and broken bones. The risk also includes catastrophic injuries such as permanent paralysis or even death from landing or falls on the back, neck, or head.
