Sponsorship Application

The 2019 Joint Professional Societies Annual Legislative Breakfast will be on
Friday, February 1, 2019 at the Palm Beach Gardens Marriott Hotel. 

To become a sponsor, click on the "Sponsorship Application" icon below to complete the
Online Sponsorship Application Form and to submit payment.
Premium ($350.00) and Regular ($250.00) Sponsorships are availabe.

Payments can be made by credit card, PayPal, or check. Regardless of payment option, please fill out the "Sponsorship Application" and select your payment type.   

Premium sponsors will be assigned a reserved table (10 seats) in the first three rows in front of the Dais, in the order in which sponsorship applications are received.
