Periprothetische Femurfrakturen bei Hüfttotalendoprothesen. Funktioneller und radiologischer Vergleich zwischen Plattenosteosynthese und proximalem Femurersatz

Zusammenfassung.

Studienziel: Die Verplattung der gleichseitigen Femurfraktur bei liegender Hüft-Totalendoprothese (Gruppe 1) wird hinsichtlich der klinischen und radiologischen Ergebnisse mit dem endoprothetischen Ersatz des proximalen Femurs mittels Tumorprothese (Gruppe 2) verglichen. Methode: Von 24 Patienten mit 25 ipsilateralen Femurfrakturen, die nach oder während Implantation einer Hüft-Totalendoprothese auftraten, konnten 13 persönlich, einer nur radiologisch (9 aus Gruppe 1, 5 aus Gruppe 2) und einer telefonisch nachuntersucht werden. 9 Patienten waren verstorben. Ergebnisse: Zum Nachuntersuchungszeitpunkt waren 4 Patienten der Gruppe 1 mit einem Stock oder keinem Gehbehelf mobil, während in Gruppe 2 alle 5 Patienten auf eine oder zwei Krücken angewiesen waren. In Gruppe 1 konnten 5 von 9 und in Gruppe 2 vier von 5 Implantaten als fest befundet werden. Schlussfolgerung: Die Verplattung zeigt bezüglich der Mobilität bessere Ergebnisse, während radiologisch der proximale Femurersatz überlegen ist. Die Patienten mit endoprothetischem Ersatz haben deutlich länger überlebt.

Aim: Clinical and radiological examinations were performed to compare plate osteosynthesis (group 1) with replacement of the proximal femur by tumor prosthesis (group 2) in patients with ipsilateral femoral fractures around a hip prosthesis. Method< Of 24 consecutive patients with 25 fractures, nine had died and one could be contacted by telephone leaving 14 hips (in one case only recent X-rays were available) for final review (nine patients from group 1; five from group 2). Results: At the time of examination four patients of group 1 were using one cane or were walking without any support, all patients of group 2 needed one or two crutches. In 5 of 9 patients in group 1 and in 4 of 5 in group 2 a firm fit of the prosthesis was observed. Conclusion: Group 1 achieved better clinical results in postoperative mobility, whereas femoral replacement showed better radiological results. Survival of patients with tumor prostheses was considerably superior to that of patients with plate osteosynthesis.
