It was announced earlier in the year that state employees can earn $100 by getting at least one dose of the vaccine and submitting a COVID-19 vaccine attestation form and a copy of the vaccination record card by Nov. 30.
Spouses of state employees can earn $25 for doing the same.
Now, there are bonus rewards of up to $1,000 for state employees.
The Ohio Department of Health said state employees could get an additional $300 and/or $600 if their agency meets the following thresholds.
To get the $300, the agency needs to have 65% of its active employees vaccinated or have received the first dose and forms are submitted by Oct. 15.
To get the $600 bonus, the agency needs to have 85% of active employees vaccinated (at least the first dose) and forms are submitted by Nov. 15.
Those who submit can expect to get their reward by Nov. 19 for the 65% goal and Dec. 17 for the 85% goal.
Employees can take up to two hours of paid leave for the flu vaccine and the COVID-19 vaccine starting Sept. 27.
