All students are required to complete this health screening form each day before participating in a school based extracurricular activity.

If a student answers YES to any health screening questions they are
NOT PERMITTED TO PARTICIPATE
in the school based extracurricular activity and are advised to visit their health-care provider.

If a student has an underlying or existing health condition which poses similar symptoms to COVID-19, the condition must be clearly stated on the student's VHSL Physical Form.

Verification of form completion will occur each day.
