THIS IS FOR PICK-UP AT OUR HOME FARM: 26533 SUMMERS RD. EAST ROCHESTER, OHIO. Pick-up is at the house, basement entrance. You can call the home phone at 330-525-5128 or contact Dale at 330-257-4495 if you need any additional service or help.

THE HOME FARM IS OPEN TUES, FRIDAY AND SATURDAY from 10am-4pm.
To avoid close contact at pick-up, select the PayPal invoice option and a secure invoice will be sent directly to the email you provide in this form to complete your payment ahead of time and we can place your order outside on a table. We ask that you complete this form no later than 5pm on the day prior to your pick-up date.Thank you for your order!
