ENROLLMENT INFORMATION
DOCUMENTS NEEDED TO ENROLL YOUR CHILD AT LINCOLN HIGH SCHOOL

1. Driver's License (must be current and match lease/mortgage, and utility bill)
2. Utility Bill* ( must be current, and match lease/mortgage and driver's license)
3. Lease/Mortgage (must be current, and match driver's license and utility bill)
4. Birth Certificate
5. Immunization Records
6. Transcripts
7. Discipline Records
8. Registration Packet

*utility bills consist of phone bills, hospital bills, pay stub, bank statement, DHS paperwork, social security etc. But must show your name and address on them.

If you live in our district but live with someone else, all documents must be provided by both you and the person you live with. The person you live with will provide the following documents:

You will provide:

IF YOU HAVE GUARDIANSHIP OR ARE NOT THE PARENT OF THE STUDENT, YOU WILL NEED TO BRING IN COURT, DOCUMENTED GUARDIANSHIP PAPERS FROM A JUDGE TO ENROLL YOUR STUDENT. AND THEY MUST BE CURRENT AND UP TO DATE. IF YOU HAVE AN APPOINTMENT TO UPDATE THOSE PAPERS, WE WILL NEED A COPY OF THAT APPOINTMENT DAY/TIME.

If you live out of district you will need to apply for SOC over at our administration building.

ONCE YOU HAVE ALL THE DOCUMENTS NEEDED TO ENROLL YOUR CHILD, PLEASE FEEL FREE TO EMAIL ME AT head.laura@vdps.net or call me at 586-758-8310. I will also be in touch to verify that you are ready to enroll your child. Enrollments will be by appointment only, so please make sure that once we make the appointment you come prepared and on time or we will have to reschedule.
