TERMS AND CONDITIONS
VETS (Veterinary Emergency Transportation Services) is a separate entity from the discharging and receiving veterinary facilty.
TRANSFERS
A VETS staff member will contact you at the time your pet is released from the veterinary clinic and provide you with the estimated time of arrival at the designated location.
The patient will be transported in a carrier, unless he/she is too large for the same or the medical condition warrants constant monitoring in which case he/she will be restrained by other means.
If the services of a veterinarian or veterinary technician are required to monitor the patient continuously during transport an additional fee applies. Please discuss the availability of a veterinary technician with a VETS staff member and indicate below.
RECEIVING MY PET AT THE DESTINATION
Pet owners are typically asked to meet us at the destination to receive their pet and ensure continued treatment, or make arrangements to contact their veterinarian by phone at the time of dropoff.
AUTHORIZATION
I am the owner, or the agent for the owner, of the animal described above, and I have the authority to execute this consent.
I hereby authorize:
The discharging veterinary facilty to release the above named pet and all pertinent paperwork, biological samples, and medications to the staff of VETS.
VETS to transfer my pet, the medical records, and any biological samples to the designated veterinary clinic.
VETS to treat my pet as advised by the discharging veterinarian during transit.
VETS to perform assisted ventilation if selected.
LIABILITY
I expressly agree to release VETS, its agents and representatives, from liability for any and all damages to my pet and to hold VETS, its agents and representatives harmless from any and all liability (except in the case of gross negligence) associated with the above-mentioned medical treatments and/or transportation being performed on my pet.
Unforeseeable circumstances such as traffic and/or inclement weather conditions may delay or cancel my pet’s transportation - I agree to release VETS from any liability resulting from such events.
REFUND POLICY
All scheduled transportation is subject to a 50% cancellation fee if the transport is cancelled for any reason within 3 hours of patient pickup time. Once our vehicle is en route to the pickup destination we are unable to offer any refunds if the transport is cancelled for any reason.
