You authorize Sandy Lake Academy to debit this bank account for all amounts authorized by you in accordance with your agreement(s) with Sandy Lake Academy for goods or services.
This agreement applies only to the method of payment between you and Sandy Lake Academy and this agreement and any termination of this agreement does not have any effect whatsoever with respect to any contract for goods or services between you and Sandy Lake Academy.
All debits to your account will be administered by our payment services provider, Helcim Inc.
You agree to waive your right to receive pre-notification of any amounts debited from your account and agree that you do not require advance notice of the amount of PADs before the debit is processed.
You have certain recourse rights if any debit does not comply with this agreement. For example, you have the right to receive reimbursement for any debit that is not authorized or is not consistent with this agreement. To obtain more information on your recourse rights, you may contact your financial institution or visit www.payments.ca.
You may amend or cancel this authorization at any time by contacting Sandy Lake Academy. For a sample cancellation form or further information on cancelling a PAD agreement, please contact your financial institution or visit www.payments.ca
