Thank you for your interest in this research study. This is a survey-based research study of organizations that are implementing OpenMRS at various locations in order to understand key metrics around its use. Please complete this survey ONLY IF you or your organization has previously used and/or is currently using OpenMRS. This could be in any form or distribution (Bahmni, OpenSRP, KenyaEMR, UgandaEMR etc.). Please fill one form per OpenMRS Deployment. If OpenMRS is being implemented in multiple countries, fill only one form and indicate the countries in the second question. Participation in this survey is voluntary. This research study is being conducted by Dr. Theresa Cullen at Indiana University.

The procedure involves filling an online survey that will take approximately 30-40 minutes. All responses will be kept confidential. De-identified, aggregate (by region) survey data will be made publicly available to the community through a report and a peer-reviewed publication. If you consent to releasing identifying information to include in the report as a case study, please select “Yes” to the LAST question.

There is no direct benefit to respondents. The findings would provide important insights around the scale and scope of OpenMRS's use, inform best practices, identify challenges in implementation, and benefit the OpenMRS community as a whole. The risks of participation involve disclosure of confidential data. We will do our best to keep your information confidential. All data is stored in a password protected electronic format. The results of this study will be used for scholarly purposes only and may be shared with Indiana University representatives.
