Title: Transforming Libraries into Community Resilience Hubs: A Pilot Project in New York’s Long Island Region

KEY INFORMATION
 (1) The information in this form is being used to seek your consent for a research study. Being in the study is voluntary. It is up to you.
 (2) This research is being done to find out the type of services that the libraries in Long Island can provide to help the public during extreme events such as hurricanes. Study procedures for this research is responding to the questions in the survey or participating in an interview if you are a library manager or staff. Survey participation should take about 10 to 15 minutes. Interview participation should take about 30 minutes to 1 hour.
 (3) The most likely risks to you of the research are:
 • There are no foreseeable risks or discomforts associated with your participation in this study. See the RISKS AND DISCOMFORTS section of this form for a complete list of possible side effects
 (4) The most likely benefits to you are:
 • There is no direct benefit expected as a result of you being in this study
 (5) If you decide not to be in the research, your choices (if any) are:
 • to refuse answering the questions You are being asked to be a volunteer in a research study.
 PURPOSE
 The purpose of this study is:
 • to list the services that are provided by libraries during the extreme events, such as hurricanes
 • to identify the needs of the public during extreme events
 • to identify strategies to improve the library services in Long Island to meet the public needs, especially for the underserved communities
 You are selected for this study because you are either a Long Island resident or a library user.
PROCEDURES
 If you decide to be in this study, your part will involve:
 • answering the questions in the survey

 The following risks/discomforts may occur as a result of you being in this study:
 • There are no foreseeable risks or discomforts associated with your participation in this study.
BENEFITS
 There is no direct benefit expected as a result of you being in this study
PAYMENT TO YOU
 You will not be paid or receive credit for your participation in this study
CONFIDENTIALITY
 We will take steps to help make sure that all the information we get about you is kept confidential. Your name will not be used wherever possible. We will use a code instead. All the study data that we get from you will be kept locked up. The code will be locked up too. If any papers and talks are given about this research, your name will not be used.
COSTS TO YOU
 There is no cost to the participant.
ALTERNATIVES
 Your alternative to being in this study is to simply not participate.
RIGHTS AS A RESEARCH SUBJECT
  • Your participation in this study is voluntary. You do not have to be in this study if you don't want to be.
 • You have the right to change your mind and leave the study at any time without giving any reason, and without penalty.
 • Any new information that may make you change your mind about being in this study will be given to you.
