By entering my name below, I agree that this is my legal signature, and that I understand and agree that neither Catawba County nor the fitness instructor shall be liable for any claims, demands, actions or causes of actions, whatsoever, to my or my child's person or property, arising out of, or connected with participation in this physical activity or fitness program. I do hereby forever release and discharge Catawba County and the fitness instructor, their elected officials, employees, or agents from all such claims, demands, injuries, damages or causes of action of any kind. The physical activity and/or fitness program has been explained to me and all of my questions answered to my satisfaction.
