At Exer More Than Urgent Care

Appointments are on a walk in basis only, no appointments will be made.

1. Go to the https://tinyurl.com/2020ahsphysicaldonation and pay the $40 donation to AHS Athletics.
2. Simply walk into the Westlake or Calabasas Exer location  from 9am-9pm. Show them your receipt from the webstore.

Any Agoura High School student who wishes to play a sport must complete the online athletic clearance process each year before they may participate (including tryouts, practices, and summer training). Online clearance for the 2020-21 school year are open and available at www.agouraathletics.org. Registration must be completed and a current physical must be uploaded on Family ID in order to be cleared. Physicals are good for one calendar year from the date of the exam.

•Must bring the C.I.F. Athletic Participation Health Form found on www.agouraathletics.org

•Parent or guardian must be present                                        

•Bring proof of the $40 payment to your appointment(printed or electronic receipt)
