After you fill out this order request, we will contact you to go over details and availability before the order is completed.  An invoice will be sent to the email provided.  Your order will not be complete until paid in full.  Boxes will be mailed at the first of each month.  Your subscription will start at the first of the following month after payment has been received. Six month subscriptions are buy 5 months get one month free. Each additional box is free shipping. A form will need to be filled out for each pet you would like to receive a box.
