4-H ONLINE ENROLLMENT FORM
Please complete the following steps to officially enroll your child into 4-H for the 2021-2022 school year. Bothnew & returning 4-H members will need to complete this process. Having your child’s info up-to-date in our system early will make the activity registration process much easier for both the parent and our staff.

This annual event teaches young people how to research, develop, and present a 5-minute “show and tell” project using posters and props on a topic that interests them. These communication skills help prepare students not only for school presentations but for life!
At the event, 4-H’ers will compete with other counties in their project area. All 4-H’ers receive a special 4-H participation ribbon and 1st, 2nd and 3rd place winners receive additional recognition. By participating, your child is eligible for various scholarship opportunities!

Are you interested in joining a 4-H specialty Club? Come check out the Columbia County 4-H Club Expo to learn more about our special interest clubs. Each club will host an info session during four 20-minute time blocks. In order to register for a club, a parent must have attended one of the club's sessions at Club Expo. Most clubs meet monthly and have very limited availability. Get there early!

More information regarding this meeting will be sent on or before October 14 to those who have registered for Cloverleaf DPA.
IMPORTANT! Each student and an adult MUST ATTEND the orientation meeting! If your child can no longer join us for the event, please let us know by October 18 so a spot can be given to another child on the waiting list. A partial refund will be available before this date.

4 -H Teen Leader Training is a curriculum-based program created to assist County 4-H programs in training and preparing students grades 7th-12th to serve as in the capacity of a Teen Leader at various events. Teen Leaders are called upon to teach workshops at 4-H events such as Junior Conference, Fall Forum, and County Day Camps and also assist district and state staff in leading and mentoring others at competitions like Project Achievement.

They’re great as a snack or to use in your grandma’s homemade pecan pie! The top 3 sellers will earn $75 4-H dollars towards summer camp or another 4-H activity!

Through a long-standing partnership with the Columbia County Board of Education, the Columbia County 4-H program is integrated monthly into every 5th-grade classroom; 95 classes total. These meetings are typically done in-person by one of our 4-H staff members or volunteers however, due to the current circumstances, we will be conducting our meetings this year through a new VIRTUAL format instead.
Each month (September - April) your child will have the opportunity to participate in a fun and engaging educational lesson focused on Georgia State Standards of Excellence enhancing classroom instruction. The best part is... these lessons are FREE and fully accessible to our Learn from Home students! If your child is in 5th grade at a public Columbia County elementary school or at SAIL Charter School, please ask his or her teacher for more details!

Would you enjoy the opportunity to participate in activities focused on topics specific to the needs of military kids and their families? None of the Columbia County 4-H clubs and activities would be possible without our dedicated volunteers! We need volunteers to help with in-school club meetings, specialty clubs, fundraisers, community service projects, and so much more!
A background and motor vehicle record check are required.

There is no official deadline but we suggest completing the enrollment process listed below by September 17. After this date, only students who have officially enrolled for this year will be sent communication regarding upcoming activities, events, trips, etc. Feel free to reach out if you have any questions!
Go to https://tinyurl.com/CC4HForms2022 to complete your child’s 2021-2022 Medical Release Form (MRF). There are other 4-H forms in this folder - you only need to complete the MRF for right now. This form has been updated recently by the state 4-H office - make sure you use the version of the form located in this folder and not one from a previous 4-H year. Please complete, sign, and date both sides of the form. The PDF can be typed into and printed but the signatures must be “wet” not digital. Once completed, the form must be scanned as a PDF (not a picture!) to ktankersley@columbiacountyga.gov.
An e-mail with the link to complete your child’s online Code of Conduct & Activity Waiver will be sent to the primary parent e-mail listed on the online enrollment card. If you do not receive this e-mail within 7 days of completing the online enrollment form, please check your SPAM/Junk folder. If you have still not received a link, please e-mail Kelli at kmcody@uga.edu.

GENERAL EVENT & CLUB REFUND POLICY
If a child can no longer participate in an event or club, all but 25% of the original fee will be refunded. Ex. Event fee is $100, $75 will be refunded. This partial refund is only available if requested within two weeks of an event or of the club's 2nd meeting. After that time, no refund will be given. All refunds are determined by the Columbia County 4-H agent.
COVID-19 REFUND POLICY
If an event or club is canceled due to COVID-19 or any other unforeseen circumstance, all but $5 of the original fee will be refunded to all currently registered participants.
The mission of Georgia 4-H is to assist youth in acquiring knowledge, developing life skills, and forming attitudes that will enable them to become self-directing, productive and contributing members of society. This mission is accomplished, through "hands on" learning experiences, focused on agricultural and environmental issues, agriculture awareness, leadership, communication skills, foods and nutrition, health, energy conservation, and citizenship.
