By checking this box the person and chapter requesting the regalia agrees to return the regalia to NPP within 72 hours of reservation date, unless otherwise directed by the NPP chaplain.

FINANCIAL INFO: There is a $40 security deposit required to reserve the regalia. This fee helps cover the cost of shipping and cleaning the regalia, and will be used in the event of any damage to the regalia. It is payable to the NPP PayPal (@KappaPsiNPP) upon submission of this form.  
Chapters or brothers responsible for shipping the regalia will be reimbursed by NPP for the cost of shipping after submitting the shipping receipt. 
ANY lost, damaged, or stolen items from the regalia set will be replaced at the expense of the renting chapter. 
If your chapter does not use PayPal, please reach out to the NPP Chaplain BEFORE submitting this form to make alternative arrangements.

By checking this box the person and chapter requesting the regalia agrees to send a copy of the outgoing shipment receipt with tracking number and shipping cost to the NPP chaplain, as well as the shipper's contact information, in order to be reimbursed for shipping costs by NPP. 
(Shipper may be a chapter or an individual brother - either can be reimbursed.)
