Thank you for your interest in participating in PHMC's study of Sesame Workshop's Growing Together program. To see if you are eligible, please answer the following questions.

5. Do you have a child in this class who is four or will turn four during the study (January- April 2020)? *This question is required.
6. During a typical week, is your child present in this classroom at least three days a week? *This question is required.

7. Do you have access to a device (e.g. cell phone, tablet, computer) with reliable, high-speed internet? *This question is required.
8. Do you have an email address that you are willing to provide to researchers? *This question is required.
9. The Growing Together Course and study surveys are only available in English. Do you read English enough to understand the course materials and/or surveys? *This question is required.
10. As far as you know, will you be available to participate in the study from January-April 2020?
(e.g. You would not be available if your child will no longer be attending this center, or if your child will be absent from the center for an extended period.) *This question is required.
