In order to participate in any UMC event produced by Odyssey Interstellar sponsors must submit the following form for each fighter they intend to participate in the UMC.

Fighters Name (RSI Handle) *

Link to the fighters RSI profile *

Sponsor Name (Organization, Fight Club, Fighting League) *

Fighters primary color *

Fighters secondary color (must be different from the primary color) *

Acknowledgement of Rules and Conditions *

We (both sponsor and fighter) have read and understand the rules and conditions for all Odyssey Interstellar UMC events and will faithfully abide by said rules and conditions. If at any time either party knowingly or unknowingly violate the rules and conditions laid out by Odyssey Interstellar we acknowledge that we may be subject to suspension or expulsion from participation in future Odyssey Interstellar UMC events.
