Meter Test Request

If a Lake Cushman water customer feels that their water meter is reading incorrectly, the customer can request that the meter be tested for accuracy. The customer must pay the deposit, as specified in the approved LCMC Fee and Fine Schedule, and complete this form before the meter is removed and shipped to a certified meter testing facility. Once the meter is removed, a certified meter will be installed. If the results of the test show that the meter is within the American Water Works Association (AWWA) tolerance of (98% - 102%), the meter is considered accurate.

I am requesting that my water meter be removed and sent to a certified meter testing facility to be tested for accuracy. I understand that a deposit is required before my application can be processed. I understand that if the test results show that the meter considered accurate; my account will not be credited the deposit and any outstanding balances will be due in full. If the test shows the meter is less than 98% accurate, LCMC shall keep the deposit. If the test results show the meter is over 102% accurate, my usage bill will be adjusted for a period not to extend back more than one service period from the date of the written request and the deposit will be credited to the customer’s account.
