Your health is our top priority. Please take 2-3 minutes each day to complete the following health screening form prior to entering the school building

Please be reminded of the following:

* If you have a temperature or if you have one of the symptoms on the questionnaire below, Symptoms of Coronavirus, you MUST stay home.

* If you become ill at school and display one of these symptoms, you must go home within 1 hour. Staff and visitors will be isolated in the health isolation room until departure.

* Notify the school nurse, at (202) 832-7370, if:  
1. Your experienced COVID-19 symptoms,
2. Exposed to someone with COVID-19, and
3. Tested positive for COVID-19.
