I apply for a FREE trial session at the London Taekwondo Falcons (LTF) and agree to comply with the rules and regulations of the LTF and London Taekwondo Academy (LTA). I acknowledge that I am aware of the nature and potential risks of and declare that I am medically fit for practising Taekwondo. I have no known injuries and agree that I will not hold the organisation (LTF) responsible for any injury I may sustain during the trail session.
