This is the official NYDKC Fundraising Report Form (FRF) for the 2022-2023 service year. Use this form every time your club hosts a fundraiser for a charity. Please note that a picture confirmation is not necessary, and note that after submitting this form, your club advisor, President, and Lieutenant Governor will receive a confirmation email (Please check spam as sometimes the confirmation email goes there). Also note that if more than one school participated in hosting this fundraiser, only one club has to fill out this form!
