Please make sure you have fully read the FHSD High School Summer School Enrollment Guide (https://tinyurl.com/FHSDSummerguide2023) prior to completing this form.  An email will be sent April 21st to the email provided in this form to confirm placement or waitlist status for Session 1.  Schedules will be available in the Parent/Student portal on May 1st.

- Non-FHSD  students will need to submit the required forms (Health and Missouri Safe Schools Act) within one week of submitting this form before the student can be enrolled.
