Please fill out additional forms for each of your children.  This information will be placed on a class list that will be used by the teacher and Class Rep throughout the year.  In addition, this information will be used for the PTO email distribution list used by the PTO and your school's main office for purposes of disseminating information about upcoming school and PTO related events. You can choose not to have your information shared with other parents in the class at the bottom of this form.
