This form is for signing up either as an individual player or a Team to compete in the Claypool Tournament on the 31st of March, 2019.  For team sign-ups, you do not need all 4 individuals to sign up, you may leave empty slots. The empty slots will be filled for you, if you do not provide an updated roster before March 29th.

Please enter a valid email that is frequently checked.  This email will be used to provide details about the event as well as a PayPal invoice for payment.  If signing up the team, it is recommended to use the captain's email.  If you are signing up as an individual player, you will receive contact information about your teammates when a 4 man team has become available.

Waivers will be available on site for you fill out.
