Due to copyright licensing and educational alignment, this form must be filled out and submitted a minimum of ONE WEEK prior to showing any movie in your classroom.  Mrs. Prather and/or Dr. Watkins will notify you regarding approval of your video request if we foresee copyright issues.

Please keep in mind that movies shown for entertainment or rewards, if approved, must be covered by our movie licensing agreement with Swank Motion Pictures, Inc.

All movies for educational purposes must be aligned to your content standards and follow our classroom video policy.  

Teachers who do not follow this process and infringe upon copyright may be responsible for fees or charges associated with copyright laws.
