Water Service Verifications                      APPOINTMENT for In-Home Material

**FOR RESIDENTS THAT WOULD LIKE TO SCHEDULE AN APPOINTMENT FOR AN AUTHORIZED CITY REPRESENTATIVE TO PERFORM THE IN-HOME INSPECTION OF THEIR WATER SERVICE LINE (WSL) MATERIAL**

The Village of Beverly Hills is redeveloping its water service line inventory and confirming the locations of any lead service lines (LSLs) throughout the City, as required by the Michigan Lead and Copper Rule (LCR). If you are reading this, you have received the letter identifying that your property has been selected to have its water service line (WSL) physically verified. 

Please take a moment to fill out this short form below to schedule your appointment. The inspection is anticipated to take approximately 15 minutes.  Hubbell, Roth & Clark, Inc., the City's Engineer, will be conducting these inspections on behalf of the City.  

A google account is required to fill out the form, if you do not have and/or do not want one please contact us to schedule an appointment.  

Please remember that utilizing this Google Form is a 100% free service and you shall not be prompted for payment from Google.
