If you plan on sending your child starting Nov 15 when we first open, or Dec 6, 2020 or Jan 3, 2021 (or potentially Mar 7, 2021), please fill out the form below.
HOWEVER, you must read the SOW Reopening Letter from PPC before you fill out this form.

I have read and understand the information sent out by Berean SOW regarding the reopening, where COVID-19 precautions are in place and I agree to comply. If you or your child refuses to comply, Berean SEEDS staff have the authority to deny admittance into the SEEDS facility. We have put in place preventative measures to reduce the spread of COVID-19 and will do our best to enforce these measures, but we cannot guarantee that an exposure will not occur. I am also agreeing to support any revisions necessary. *
