Student Advocacy Office Privacy and Confidentiality
PRIVACY AND CONFIDENTIALITY:
All information submitted to the Advocacy Office will be held in strictest confidence and no action will ever be taken without your consent. Information provided to the Advocacy Office will not be shared with SFU, the SFSS or any other parties without your consent, unless required by law or if, in the opinion of the Advocacy Office, compelling circumstances exist that could affect anyone’s health or safety. The Child, Family, and Community Service Act (CFCSA) also requires that anyone who has reason to believe that a child or youth has been or is likely to be abused or neglected, and that the parent is unwilling or unable to protect the child or youth, must report the suspected abuse or neglect to a child welfare worker.

Please note that the information submitted through this form will be saved for the purposes of internal case management, issue tracking and longitudinal statistical data collection. Identifying details (name, student number etc) will remain attached to the information you provide while your case is open. Data generated using this form will be securely stored on Google servers located in the United States, (not SFU servers). All reasonable efforts to protect and secure the information you provide will be undertaken.

Should you wish to anonymize your responses to this form for privacy purposes, please use a pseudonym, omit your student number, and inform the Advocate of this when you discuss your case.

If you do not wish to have your details stored in the manner, please speak with the Advocate (the Advocate's office is SUB 3301) or email studentadv.coord@sfss.ca to arrange an alternative.
