Athletic Sign-Up for Spring 2023-2024
Please fill out the following form if your student is interested in participating in our spring athletics program.  This includes Softball and Baseball.  This is open to all of grades 5-8 to try-out for the team.  The following requirements must be met prior to try-outs or the beginning of the season.  Try outs will begin mid-March.
-  Students must be in good academic standing (a 2 or above in ALL classes and caught up on all work)
-  Have had minimal office referrals or disruptive behavior (detentions, suspensions, etc)
-  Students must not fall into the excessively absent category (20% of the school days to date)
-  Must have an updated sports physical on file within the last two years (if you turned one in for fall or winter athletics you don't need to submit again). 
-  Must have the extra-curricular agreement signed (this only needs to be done once a year)
-  Must have a concussion for signed (this only needs to be done once a year)

Please note that we will look at team sizing and how many athletes we can reasonably support on the teams when making decisions about cutting.

These only need to be filled out once per year and do not need to be filled out if your student participated in fall athletics.
