According to The Student Code of Conduct, hazing means doing any act or coercing another, including the victim, to do any act of initiation into any student or other organization or any act to continue or reinstate membership in or affiliation with any student or other organization that causes or creates a substantial risk of causing mental or physical harm to any person, including coercing another to consume alcohol or a drug of abuse, as defined in section 3719.011 of the Ohio Revised Code.
Victims or witnesses of hazing-related situations may not want to talk to anyone about their experience for a variety of reasons. However, we want to ensure that the University has important information regarding potential threats to the safety and well-being of our students and campus. Using this form, you can submit an anonymous report to the University, which will be sent to the Office of Community Standards & Student Conduct for review and follow up. Please note that our ability to act on anonymous reports is limited by the amount of information we receive. This form will allow users to enter their name and contact information, if reporting parties choose to self-identify during the submission process.
