   - Emergency contacts
   - Names of people who can pick-up your children
   - Yes/No for photo release for your children

SUBMIT SEPARATELY Oct 6, 2019:
  - IEP or 504 info (if needed)
  - Grades K, 6, and New students: Official immunization history from a physician, OR download and complete our similar (then signed by your child's physician) form: https://tinyurl.com/2019btcImmunization

PARENTS will sign the following appropriate agreements NOW, and STUDENTS will need to sign them in the fall:
   - 5th, 6th, and 7th Graders - B’Nai Mitzvah Policy
   - 9-12th Graders - Gesher L'kesher letter and Code of Conduct

PARENTS will be asked which Religious School volunteer opportunities they would be able to help with.

FEES will be paid starting in September 2019
   $60 - ALL Students - Materials fee
   $491 - Pre K, K, 1st Grade
   $737 - 2nd-7th Grades
   $737 - 8th-12th Grades - Hebrew Option (NOT for Credit)
   $624 - 8th-12th Grades - Non-Hebrew Option
   $150 - 8th-12th Grades - Dinner with the Rabbi
