Release of Liability The above child(ren) desires to participate in various programs, events or activities in the United States (hereinafter collectively referred to as the Activities) operated or sponsored by Westwoods Community Church (hereinafter referred to as WCC). I understand and acknowledge that WCC will not allow my child(ren) to participate in the activities without the releasing and holding WCC harmless from any liability arising out of my child’s participation in the activities. I have investigated the risks involved in my child’s participation in the activities and fully understand and assume such risks. I REQUEST THAT WCC ALLOW MY CHILD(REN) TO PARTICIPATE IN THE ACTIVITIES, AND IN CONSIDERATION THEREOF AGREE HEREBY TO RELEASE AND FOREVER DISCHARGE WCC, ITS OFFICERS AND DIRECTORS, AND ITS EMPLOYEES, AGENTS, AND ANY PARTIES VOLUNTEERING ON BEHALF OF WCC, FROM ALL ACTIONS, CAUSES OF ACTION, INJURIES, CLAIMS, DAMAGES COSTS OR EXPENSES OF ANY KIND, GROWING OUT OF OR RELATED TO ANY SUCH ACTIVITIES IN WHICH THEY PARTICIPATE. Photo Use Policy Westwoods Community Church will not publish any image of a minor unless his or her parent or legal guardian has signed a permission form, nor will Westwoods Community Church publish any names associated with the photos unless further permission has been granted. I hereby grant permission to Westwoods Community Church to use my image in any and all of its publications without further consideration. Permission is also granted to use images of my children listed below. I understand that no names will be used unless specific permission, in written form, is given to an appropriate staff member. I understand that once any image is posted o the Westwoods Community Church’s web site or Facebook page, the image can be downloaded by any computer user around the world. I hereby attest that I am the legal parent/guardian of the child(ren) listed below. *
