If you wish to resign your membership, please check the box below. You must also send the completed form <http://tinyurl.com/colonymembershipresignation> with a web signature postmarked by March 15 in order to receive your full membership certificate refund. Requests received after this date, but before April 15 will forfeit $50 of the certificate refund. Request received after April 15 will forfeit $100 of the certificate refund.
