WISAY Mentoring Match Form
We will use the information that you provide to us to pair you with your mentor and/or mentee.  You can expect to hear from us in October with the matches.  If you have questions before then, please feel free to email us at wisay@yale.edu.

We will do our best to match your mentor/mentee to your interests as closely as possible. However, due to a shortage of members, mentoring pairs are not always exact matches. If you find you and your mentor/mentee are irrecoverably mismatched, please let us know and we'll do our best to fix things.

Are you the first person in your family to pursue higher education or graduate study? Do you have or want children? Are you interested in training or working internationally? Are you a member of another minority (race, religion, sexual orientation, gender identity, etc.) that further affects your experience in science? Are you negotiating career plans with a partner who is also working in the sciences or academia (i.e., the "two-body problem")? All answers are optional here; feel free to share only what you feel comfortable sharing.
