Monitoring Form
Milk Presents is committed to ensuring everybody can access and enjoy the arts, and have equitable access to queer spaces and opportunties. We know that the UK theatre industry has a long way to go to engage with and represent the full range of experience of the people living in this country. We want to do what we can to change that. To achieve this, it is useful for us to know a certain amount about who you are.
We will also use this information to find out who we work with, and who we don't. It will allow us to advocate for what we do, for example sharing information with funders, and to also see where we have more work to do.
The information that you give us on this form will be treated confidentially and stored in an anonymised form. The form itself will not be retained and will be destroyed. The information will be used only to help us monitor the people we engage with and to enable us to continue to do so and improve where needed.
The personal information you supply below will NOT be made available to anyone in any form other than anonymised unless we expressly ask your permission.
Submission of this form indicates your explicit consent that your data may be processed in accordance with the provisions of the Data Protection Act 1998 and Milk Presents’ Protection Policy.
