12th Annual Hustle & Heart Basketball Clinic to Benefit Danny Did

Enjoy your day off from school at a 90 minute basketball clinic for boys and girls, hosted by volunteer coaches and players from the best high school basketball programs in the area. UPDATE: online registration is now closed. Walk up registration for session 1 will be available on site for $40 per child. Session 2 is completely sold out.

Date: Monday, February 19th (School Holiday)

Venue: Golf View Recreation Center, 7800 N. Caldwell (Niles Park District)

Cost: $30 per child. This early bird price increases to $35 per child on February 5th.

Sibling Discount: If you have more than one child attending, the 1st child is $30 and $25 for each additional child (early bird rate). Please register your first child at the normal ticket price and then add your additional children using the "sibling" ticket price. On February 5th, the sibling pricing changes to $35 for the 1st child and $30 for additional siblings.

A t-shirt is included for each participant.

Registration: Online only. You will list the parent's name, email & cell, as well as each child's name.

Format: Basketball players from partner high schools will volunteer as counselors for this clinic. Each session will include basketball skills and drills, and fun contests & relays, plus information on epilepsy and seizure safety. All ability levels are welcome. Pick one session when registering.

• Session I: 9 am – 10:30 am (Kindergarten – 8th Grade)
• Session II: 10:45 am – 12:15 pm (Kindergarten – 8th Grade) UPDATE: NOW SOLD OUT

Each player must have a signed waiver to participate. You can read the waiver below and agree to the waiver during online registration.

The clinic will be led by coaches and players from: Loyola Academy (Boys & Girls Teams) Notre Dame College Prep, Benet Academy, St Patrick High School, Depaul Colleg Prep, Truman College, Lane Tech, and Regina Dominican.

WAIVER AND RELEASE OF LIABILITY, ASSUMPTION OF RISK, HOLD HARMLESS AND INDEMNIFICATION

As a condition and in consideration of our Child’s participation in The Hustle and Heart Basket Ball Clinic (“Clinic”), we acknowledge, understand and agree as follows:

The Child(ren) listed with this registration (“Child”) is/are the Participant(s).

As a condition and in consideration of our Child’s participation in The Hustle and Heart Basket Ball Clinic (“Clinic”), we acknowledge, understand and agree as follows:

1. The risk of injury from the activities involved while participating in the Clinic is inherent and significant, including the potential for injury, permanent paralysis and death;

2. My Child is participating voluntarily in the Clinic and I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility and any resulting financial obligation if my Child is injured, for my Child’s voluntary participation in the Clinic;

3. I willingly agree to comply with the stated and customary terms and conditions for participation in the Clinic. If however I observe any unusual significant hazard during my presence or participation, I will remove my Child from participation and bring such to the attention of the nearest Clinic official immediately;

4. I, for myself and on behalf of my Child, heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, AGREE TO INDEMNIFY AND HOLD HARMLESS The Danny Did Foundation, their owners, its officers, directors, sponsors, volunteers, and/or employees, participants, and the Stanton Family, WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE AND ANY CLAIM FOR PROPERTY DAMAGE OR PROPERTY THEFT. I HAVE THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

5. This Waiver And Release Of Claims, Assumption Of Risk, Hold Harmless And Indemnification Agreement shall be construed broadly to provide a release and waiver to the fullest extent permissible under applicable law.

6. PRIVACY AND PUBLICITY. I further understand that my name, photograph, voice or likeness may be used by Danny Did Foundation without compensation. I consent to and authorize, in advance, such use and waive any rights of privacy and/or publicity I may have in connection therewith.

FOR PARTICIPANTS OF MINORITY AGE (under 18 at time of registration)

This is to certify that I, as a parent or legal guardian with legal responsibility for this Clinic participant, do consent and agree to his/her release as provided above of all the Releases, and, for myself, my heirs, assigns, and next of kin, I release and agree to indemnify the Releases from any and all liabilities incident to the minor Child’s involvement or participation in the Clinic as provided above.
