Your responses are completely confidential: no one will be able to link your responses to you. Please answer all of the questions, but if you prefer to skip one, you can still continue with the survey. The questions contained in the survey are based on topics that were raised over the last year during a series of town hall meetings, focus groups conducted with parents, young people, teachers, and clinicians, and interviews with local officials and agency directors and board members.
