Thank you for taking the time to help us with this study. You will be asked to answer a few survey questions. The survey is not long, and should take you less than 20 minutes to complete.
ABOUT THE PROJECT
This study will be used to assess the quality of services and programs at the Lighthouse of Houston.
To begin, you will be asked to tell us some things about yourself. These are questions about your age or whether you are male or female. Later in the survey, you will be asked about some of the programs and services offered at the Lighthouse. For your privacy, we will not ask you to give us your name.
The answers you give mean very much. They will help the Lighthouse better serve its clients. Your answers may also help other blind and visually impaired people.
INFORMED CONSENT
There are a few things you should know about helping with the study:
• First, being a part of the study is up to you. You do not have to answer every question. You can stop at any time and for any reason, but every answer is helpful.
• Please be honest when answering the questions. None of your answers will be linked back to you. All that you share will be added to the answers given by the group. You will also not be singled out in any future reports or uses of this research.
• Nothing about you, or anything you tell us, will be shared without your consent, unless the law calls for it.
• You have the right to know the possible risks that may come with your being in this study. There is very little risk. Possible psychological risks, like concerns about your privacy, should be small and rare.
