Penguin of Concern Referral Form


The Office of the Dean of Students, along with the YSU CARE Team (Concern – Assessment – Referral – Education) Team, address concerns that may negatively affect the campus learning environment or potentially harm the health, welfare and/or safety of members of the University community or the individual(s) exhibiting such behaviors. Referrals are accepted from students themselves, their families, faculty, staff, and the surrounding community with the goal of providing outreach, guidance, and assistance to students dealing with a variety of issues. 

This referral serves to address concerns related to the following: 

  • Vague/Direct Threat
  • Mental Health
  • Behavior/Conduct
  • Sexual Assault/Misconduct
  • Personal/Social
  • Food/Housing/Financial Insecurity
  • lllness/Hospitalization/Other Medical Condition
  • Student-Athletes
  • International Students

* Academic issues should also be directed to the Resch Academic Success Center.

* Issues involving a faculty or staff member should be submitted using the Univeristy's Student Complaint process.

NOTE: All members of the YSU community have the responsibility to report any situation that could possibly result in harm to anyone at YSU. However, it is important to understand that the Office of the Dean of Students and the CARE Team should not be confused with crisis management. In a crisis situation, where there is an active or immediate risk of violence to self or others, please contact the YSU Police Department immediately at 330-941-3527 (or dial 911 from a campus phone).
